FORMS/WAIVERS  In order to purchase a Discover McDermont or All-Access Day Pass, and to participate in the attractions at McDermont Field House, you must sign a waiver. Download the General McDermont Field House Waiver by CLICKING HERE for English or CLICKING HERE for Spanish. Download the Screamin' Eagle Zipline Waiver by CLICKING HERE. If you are bring a group of people or a party, please fill out the Party/Group Waiver seen HERE. In order to register a Team for a sports league held at McDermont, please fill out the attached TEAM REGISTRATION FORM. In order to register as an Individual for a sports league held at McDermont, please fill out the attached INDIVIDUAL REGISTRATION FORM.  Are you looking to rent the Lindsay High School soccer/football fields? The FIELD RENTAL AGREEMENT is now available.
